application form completely; if you are unable to answer a question,
please enter "not yet known" or give us an estimate. Within
24 hours of receipt, we will send a confirmation email, letting
you know that we have your application. The length of time it takes
for us to make a funding decision varies a great deal. If you have

If this is a life threatening emergency and
you need a decision from us within 24 hours, then YOU need to make
sure we are provided with the following:
The vet must be available to speak with an OSLF
representative. Carefully read through the information on our Emergencies page, and follow the step-by-step procedures outlined there.
In the medical section of your application, please
give us as much detail as you can on the nature of the emergency
treatment needed and information on what treatment has already been
given.

IMPORTANT! Please be sure you have your application package complete before submitting any paperwork for review. All supporting documentation must accompany your application and all paperwork must be submitted at the same time in order to be eligible for review. If all paperwork is not received together, the application package will be considered incomplete and the application will be denied. This includes all veterinary clinic paperwork. See the Supporting Documentation Checklist at the bottom of your application form for a list of required documentation.
You will also need to print out, and return a copy signed by your veterinarian, of OSLF's Letter of Introduction and Veterinarian Waiver [PDF].
OSLF should be considered the last resort source of funding. If you have other resources, or are eligible for other funding through a breed or other rescue organization, those resources must have been denied or exhausted before applying to OSLF.
